Day Habilitation Specialist - 35 Hrs/wk (GSBDH)

Shift: Monday – Friday, 8:30A – 4:30P

Qualifications:

• Associate degree
• Clean and valid NYS Driver’s License required
• One year of experience working with individuals with developmental disabilities
• Computer skills
• Bachelor’s Degree preferred, but not required

Responsibilities include, but not limited to:

• Oversee caseload of approximately 40-45 individuals.
• Responsible for following-up on all individual issues and concerns.
• Proficient in the use of an electronic record keeping program.
• Develop Staff Action plans based upon individual choice as identified in Annual Team meetings and family/guardian input.
• Develop valued outcome/informal outcomes and input into Therap.
• Collect and review data to write progress for monthly notes.
• Organize and conduct annual and semi-annual review meetings.
• Provide guidance and counseling to individuals
• Maintain contact with families/guardians/residences/Care Managers to ensure seamless delivery of services.
• Ensure compliance with OPWDD regulations and Day Habilitation Policies and Procedures, including but not limited to training on behavioral plans/strategies and protocols.
• Ensure needs of individuals are met including but not limited to their safety and well-being.
• Ensure individual appointments and schedules are adhered to.
• Attend and participate in staff meetings, agency committees, trainings and seminars.
• Incorporated Code of Ethics, Core Competencies and Empowerment Strategies in their daily professional responsibilities.

Non-essential job functions:

• Responsible for meeting all the personal care needs of individuals with developmental disabilities, including but not limited to feeding, transferring, lifting, toileting, dressing (coats), etc., as per the individuals’ protocols.

The Cerebral Palsy Association of Nassau County is a nonprofit, multi-faceted health agency serving over 1,500 children and adults with cerebral palsy, developmental and other disabilities.

Day Habilitation is a year-round program serving individuals ages 21 and older who are developmentally disabled. Day Hab is a community outreach program where trained and qualified individuals with developmental disabilities provide local non- profit businesses with community service. It is a comprehensive program which focuses on community integration and volunteerism.
