The Designated Coordinator provides supervision, support, and evaluation of activities to promote independence for individuals with disabilities receiving services through Blue Sky, Inc.
ESSENTIAL DUTIES AND RESPONSIBITIES:
- Create plans for participants that include person-centered goals and generate strategies that will guide direct support staff in helping each participant to succeed in reaching those goals.
- Ensure participants are reaching their goals and work with their teams to find creative ways to help them succeed in all areas of life.
- Mentor and coach direct support staff so they can learn exactly how to support our participants when working on their goals and in community integration; and
- Make necessary changes and revisions to plans as the participant achieves their goals to support further growth in each individual we serve.
MINIMUM Requirements:
The Designated Coordinator must minimally meet one of the following:
- a baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older OR…
- an associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older OR…
- a diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older OR…
- a minimum of 50 hours of education and training related to human services and disabilities; and
- four years of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).
