Human Services Coordinator I Job at DMR-Western Region
The Department of Developmental Services (DDS) is seeking an experienced, knowledgeable, and motivated Human Services Coordinator I (HSC I) to work in the Worcester Area Office.

The HSC I oversee the safety and wellbeing of assigned individuals and advocate in regard to their human, civil, and legal rights through the implementation of support services, the Individual's Service Plan, and other necessary means. The HSC I will monitor DDS funded and auxiliary funded programs by visiting the establishments on a frequent and as needed basis as well as documenting the visits and observations as regulated by the Department of Developmental Services. Regular contact with individuals, families, and providers is a necessity for cohesiveness to work with the team.
The selected candidate will be required to travel to individuals' homes, programs, and outside meetings that may or may not be in close proximity of the Worcester Area Office. It is essential for the HSC I to be responsible for the coordination, facilitation, monitoring the development and progress of support plans for assigned individuals as per DDS regulations, as well as coordination of DDS funded and auxiliary support services in response to individual's needs. The ability and willingness to utilize the communication method of individuals assigned to a caseload is an important function of this position.
The HSC I will also be required to coordinate referrals to community services and other state agencies, as well as provide outreach and communication with these entities. The selected candidate must demonstrate a willingness to work with individuals and families interested to participate in the Participant Directed Program. This will involve training to learn and perform the duties of a support broker to assist families and individuals in creating specialized support services.
The Human Services Coordinator I must maintain up-to-date record keeping – both electronic and hard copy, maintain appropriate and timely documentation for individuals enrolled in any of the Department's waiver or non-waiver services, communicate with team members, as well as maintain regular communication with the area office management team. It is essential to effectively represent the Department of Developmental Services and the Worcester Area Office in a collaborative and professional manner. Other employment responsibilities may be assigned to meet the needs of people served through the Department.

This position is located in the Worcester Area Office in Worcester, MA
Schedule: Monday-Friday, 9:00am – 5:00pm
Days Off: Sunday/Saturday
Flexibility required to meet the needs of the office.
