Summary

Under the direction of the Program Manager and respective Program Coordinator, the case manager will implement linkage and individual rehabilitation services for participants of SSG Weber Community Center. The case manager will provide office or field based individual, group and family needs assessments, linkages, individual rehabilitation and follow up services. The case manager will also conduct outreach services to promote agency services.

Essential Functions

• Conduct needs assessment and client service plan that meets individual needs, securing needed services for participants, and meeting with participants regularly to discuss their progress.
• Participate in weekly case conferencing with treatment team.
• Provide comprehensive, professional case management services to participants through encouraging, promoting and developing positive life skills and meeting emergency and long-term participant needs.
• Conduct outreach services to identify and engage potential participants.
• Conduct outreach to housing networks and employment and life skills development networks where linkages can be created.
• Maintain up-to-date and accurate documentation in client files
• Collaborate with our partners as well as any other supports in the community.
• Complete all documentation within the standards of the Department of Mental Health in a timely manner
• Maintain a minimum of 70% billing productivity monthly
• Assist with the completion of necessary client documentation (i.e. IHOMES, consent forms, etc)
• Maintain files/records on client services in compliance with HIPAA and other funding requirements for auditing purposes.
• Work on other appropriate tasks as assigned by Supervisor and Managers
• Other duties as needed.Secondary Functions

Minimum qualifications

• Verification of Employment Eligibility and Background Check.
• California Driver License, Proof of Car Insurance, Reliable transportation.
• Bachelor’s Degree in social work, psychology, human service or related field from an accredited or state approved college or university with 1+ years of human services experience (prior case management experience in a social service setting is highly desirable). In lieu of Bachelors, an Associate’s degree with two years experience, or high school diploma with a minimum of 4 years experience.

Environmental Conditions (Working Conditions)

• The environment for this position is field-based as well as an office environment. It involves working within at-risk areas including home and school based services. Exposed to potentially highly aggressive clients.

• Typically spends time sitting, standing, walking, driving, carrying (max. 20lbs), listening, speaking.

• The incumbent in this position must be able to handle any/all of the following: constant distractions, interruptions, uncontrollable changes in priorities/work schedules; be able to process information, think, and conceptualize.'
