JOB SUMMARY: Under direction of the RN Team Leader, participates in the planning, implementation, and evaluation of nursing care, within the objectives, standards and policies of the department. The Nurse Partner, LPN will collaborate with the inter-disciplinary team and strive to provide nursing services in a compassionate, professional, ethical, and knowledgeable fashion. Leads by example, maintaining a professional work relationship with team members, Residents, Clients, Members, Patients, and family members. Remains knowledgeable of residents’ rights, including the right to determine their own “Daily Pleasures.” Personally responsible and held accountable for following Fellowship Senior Living’s mission, culture (Fellowship Spirit) and values while maintaining and living our philosophy of person-directed care.

MINIMUM ACCEPTABLE QUALIFICATIONS AND REQUIREMENTS:

  • Graduate of an accredited school of Licensed Practical Nursing. Current, unencumbered professional license in the State of New Jersey.
  • Minimum of one year experience in a health care setting, preferably hospital, or long-term care facility, preferred.
  • CPR/AED certification.
  • Specialized training or experience in geriatric nursing is desirable.
  • Demonstrated ability to supervise Care Partners (CNA/CHHA).
  • Must demonstrate the ability to work within a team setting and possess strong interpersonal relationship skills.
  • Knowledge of federal and state regulations governing clinical care and nursing practice.
  • Commitment to person-centered and resident directed care is essential.
  • PC literate and demonstrated knowledge of selected software programs.
  • Excellent written and oral communication skills in English.
  • Ability to respond to and work with a mature resident population, contributing to a warm, caring and supportive environment for the enjoyment of the residents/clients and to promote this philosophy to all team members.
  • Understanding and support of the mission and philosophy of Fellowship Village, Inc. Supports the goals of the Board of Directors and the administration.

WORKING ENVIRONMENT:
Nurse Partner (LPN) functions independently, has flexibility, and demonstrates personal integrity when working effectively with residents, families, and team members throughout the organization. Nurse Partner (LPN) may be exposed to infectious waste, diseases, or conditions. Nurse Partner (LPN) must observe guidelines to guard against communicable diseases, accidents, and other dangers, wearing personal protective equipment if applicable. Nurse Partner (LPN) may be subject to stressful, uncomfortable, and/or emotional situations/individuals.

PHYSICAL DEMANDS:
The physical demands described here are representative of those that must be met by a team member to successfully perform the essential functions of this job. Work requires physical activity and moderate movement throughout the workday in order to perform daily duties. While performing the duties of this job, the team member is frequently required to stand on his/her feet for a long period of time; bending and extensive walking; use hands to grip or feel; reach with hands and arms; push and pull a med cart and other medical equipment; and talk or hear. The team member is occasionally required to reach, stretch, stoop, kneel, or crouch. Specific vision abilities required by this job include close vision, distance vision, and depth perception. Requires the ability to lift a maximum of 35 lbs. Must use the aid of an assistive device or get assistance when lifting or moving heavy objects.
Nurse Partner (LPN) must use universal precautions in all situations. In the event there is a need to evacuate the building, heavy lifting may be required to carry residents to safety. Must be able to operate office equipment (phone, computer, printer, copier, fax machine, etc).
COVID-19 considerations:
The Pines requires all employees to be full vaccinated against Covid-19 and be able to show proof at the time of hire. Per New Jersey Executive Order 283, all employees working in a long term care setting must be "Up to date" on all vaccinations including any booster shot for which you are eligible per NJ department of health guidelines. Pines Village, in compliance with the NJ Department of Health, require all team members be vaccinated against the flu or have a valid medical exemption from your personal physician.
