Harmony Home Health seeks a Full Time or Per-Diem Licensed Vocational Nurse to join its dynamic team of healthcare professionals in the West Contra Costa County/East Bay. Harmony Home Health is Medicare-Certified and State-Licensed.

The responsibilities of the Licensed Vocational Nurse include:

• Provides direct patient care as defined in State’s Name Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor/Nursing Supervisor.
• Uses equipment and supplies effectively and efficiently.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the registered nurse.

Minimum qualifications of the Licensed Vocational Nurse include:

• Active Licensed Vocational Nurse License in the state of California - at least 1 year professional LVN experience required
• Home Health (LVN) experience is a plus
• Able to prioritize and manage time effectively.
• Highly organized and detail-oriented.
• Able to follow verbal and written instructions
• Able to communicate effectively. Bilingual communication a plus.
• Dependable Transportation, Current Driver’s License and auto Insurance.
• Current CPR
• Ability to perform the physical aspects of the position.
• Complete TB test and Physical Examination
