POSITION TITLE: Life Skills Case Manager
RESPONSIBLE TO: Youth Outreach Director

RESPONSIBLE FOR: The Life Skills Case Manager general responsibilities are: 1) Develop and implement a comprehensive life skills program for runaway, homeless, and at-risk youth. 2) Conduct case management with homeless youth in the Transitional Living Program (TLP). 3) Assist with outreach and provide or connect youth to basic need supports (food, hygiene products, clothes), counseling, academic support, jobs assistance, case management, and enrichment activities. Schedule: Monday- Friday and 9am to 6pm. Occasional evenings and weekends may be required.

ESSENTIAL DUTIES:

  • Create a positive team atmosphere providing quality service to clients, staff and volunteers through active listening, positive problem solving, and timely responses.
  • Implements and participates in professional-level runaway homeless youth programs that requires contact with clients and community.
  • Responsible for ensuring case management and shelter activities correlate with established goals and objectives.
  • Requires completion of specialized training that meets Family Youth Service Bureau (FYSB) requirements, including but not limited to crisis intervention and de-escalation, human trafficking, suicide prevention, harm-reduction, and trauma-informed care.
  • Develop and coordinate life skills classes such as budgeting, household activities, job readiness, anger management, social skills, and study habits.
  • Conduct case management with homeless youth in the Transitional Living Program (TLP) age 16-21.
  • Provide on-going support for runaway, homeless, and at-risk youth including developing and monitoring client goals, advocacy, referrals to community resources, and outreach.
  • Refer youth to appropriate community resources and provide follow-up monitoring and support.
  • Provide or connect youth to basic need supports (food, hygiene products, clothes), counseling, academic support, jobs assistance, case management, and enrichment activities.
  • Develop and implement quality age-appropriate skill groups, activities, and programs for the Drop-In

Center on a regular basis.

  • Maintain current documentation and records as required for program evaluation and reporting purposes and ensure all files and reports are accurate and meet grant standards.
  • Serve as “after-hours” on-call staff as needed to ensure participant monitoring and timely crisis response.
  • Support street and school outreach to identify and recruit program participants.
  • Collaborate with other staff, agencies, businesses and volunteers to ensure that agencies’ programs coordinate to provide the best possible services for youth and wise use of resources.
  • Expand agency network and resource development opportunities through community contacts, training, promoting agency programs and involvement in cooperative activities with other agencies, groups, or organizations.
  • Drive to and from locations for outreach, and transport youth to and from school, appointments, or other essential needs.
  • Other duties as assigned by Program Director.

KNOWLEDGE, SKILLS AND ABILITIES

  • Bachelor’s degree in Social Work, Education or related field preferred (or extensive work experience)
  • Strong interpersonal skills for connecting and working with youth from diverse backgrounds (including, but not limited to, ethnic, religious, physical, and mental abilities, etc.)
  • Experience in case management or providing direct services to at-risk youth, ages 11-21 and their families preferred
  • Skill in verbal and written communications, problem solving, and team work
  • An understanding of at-risk, runaway, homeless, and street youth issues
  • A familiarity with youth support services in Yamhill County
  • Bilingual- (English and Spanish) and bi-cultural experience and abilities preferred
  • Recognition of appropriate boundaries and the ability to implement them
  • Ability to work effectively in stressful or crisis situations
  • Ability and willingness to work under pressure and on multiple projects in a timely fashion
  • Strong oral and written communication skills
  • Commitment to work as an essential part of a team
  • Demonstrated ability in the implementation and development of quality youth programing
  • Computer experience
  • Ability to be flexible in work schedule
  • Ability to pass drug/alcohol & background check and insurance check of driving record

PHYSICAL DEMANDS
The physical demands described represent those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee may be required to sit, stand, kneel, walk and/or bend for extended periods of time. May lift and/or move up to 20 pounds.

Job Type: Full-time

Pay: $20.00 - $23.00 per hour
