LICENSED VOCATIONAL NURSE JOB DESCRIPTION – HOUSE

Qualifications:

Must maintain a valid and current California Vocational Nurse License

Valid CPR/First Aid Certification

General Description:

Participates in providing nursing and health care services to the clients according to the policies and procedures of the facility and within the standards of the California Nurse Practice Act.

Duties and Responsibilities:

· Participate in and/or conduct in-service education training.

· Assist in the implementation and continuing evaluations of each of the client’s treatment plan.

· Maintain the security and cleanliness of all medication storage areas.

· Assist in the maintenance of client medical records to assure complete, accurate and pertinent information of all health conditions.

· Assist in the ongoing medical/clinical assessment of health conditions by taking, recording and reporting vital signs and other clinical observations.

· Provide and document nursing care to clients as needed.

· Ensure client safety at all the times.

· Uses and practices basic assessment (data collection), participates in planning, executes interventions in accordance with the care plan or treatment plan, and contributes to evaluation of individualized interventions related to the care plan or treatment plan.

· Provides direct client care by which the licensee:

1. Performs basic nursing services as defined above

2. Administers medication according to the regulations and Angel’s View policy.

3. Applies communication skills for the purpose of client care education.

4. Contributes to the development and implementation of a teaching plan related to self-care for the client.

5. Provides staff training for restricted health care procedures for the clients to be implemented.

6. Learn and document all necessary incident reports and nursing notes on the company system Therap.

RE: Assessment - The LVN may perform basic physical “assessment” and basic mental “assessment” defined by the BVNPTE as data collection. Basic skills for physical assessment include observation, auscultation, palpitation and percussion. Basic mental status assessment include observation of general behavior, appearance and attitude, collection of data relative to sensorium or orientation to time, person, and place and assessment of recall ability.

When the licensed nurse notes a problem, complication or abnormality, he/she must notify the registered nurse consultant under whose direction the licensee functions. The licensee cannot independently evaluate or synthesize data. As such, the LVN cannot independently formulate a nursing care plan, make a nursing diagnosis, or decide a plan of action. However, the LVN can assist with all of the above. In addition to performance, licensees are to document their findings.

Notify the RN and Physician of changes in client’s condition. Transcribe and implement Physician orders as per facility policy.

Assist with client repositioning/ transfers as needed.

Assure prevention and or early identification or client health needs by making frequent client observations, nothing health needs and conditions, an initiate reporting /documentation process of medical intervention.

Participates as a member of the interdisciplinary team

Monitor client hygiene.

Follow up on lab x-ray orders, obtain results and report any significant change to the RN.

Assist the RN with the monthly review of New Medication Administration Records and routine renewal of drug and diet regimen orders.

Monitor all client immunization requirements and assist in the coordination of delivery and administration of needed vaccines.

Assist in scheduling outside appointments for Clients, arrange transportation and prepare needed information to provide continuity care.

Upon completion of ventilator Management Training, will monitor, and or adjust the ventilator settings for ventilator dependent for clients who are stable.

Assist direct care staff with client care.

Perform all other job related duties as assigned by the RN, Administrator or Facility Manager.

Pay: $26.00 per hour
