PRINCIPLE DUTIES AND RESPONSIBILITIES

· Directs and supervises the daily work assignments of the HHA/CNAs

· Coordinates care needs with community providers via case management process

· Balances service demands with supporting resident independence

· Accurately assesses, communicates, and documents residents'/patient's status

· Observes and reports any significant changes in resident/patient behavior and health to the Resident Care Director, physician, family, HHA/CNAs and

· Responds to emergencies and personal emergency response system and knows when to call for backup

EDUCATION / EXPERIENCE / LICENSURE – CERTIFICATION

· Current state license as an RN/LPN and CPR certification

· Graduate of an approved RN/LPN program

· Minimum of 1 to 2 years of experience preferred
