The Family Intervention Program (FIP) is located at the Visalia Youth Services South Clinic. FIP is a program contracted between Turning Point of Central California and Tulare County Health and Human Services Agency Tulare Works. FIP traditionally provides short term child mental health services throughout Tulare County for Tulare Work’s participants in Visalia Youth Services South clinic, Porterville Tulare Works office, school, Resource Centers, client home, and other community locations.

JOB SUMMARY

The Personal Service Coordinator/Case Manager II reports to the Program Director and Assistant Program Director to provide mental health and case management services to consumers in the Family Intervention Program. The Personal Service Coordinator/Case Manager II will work under the direction of clinical staff to ensure that the services outlined in the contract/agreement are met. These services are deemed short-term (up to 6 months duration) with the goal of assisting children/families/parents with removing and reducing barriers to employment. The Personal Service Coordinator/Case Manager II will provide services which will assist individuals in gaining access to needed Medi-Cal eligibility, medical, social, housing, educational and other services. Will also provide mental health clinical services to consumers in other Turning Point Children’s Mental Health Program’s on an as needed basis.

ESSENTIAL JOB RESPONSIBILITIES

  • Provide direct mental health services to/for participants in the Family Intervention Program at TulareWorks sites, in the community, including individual, family and group treatment, case management, and consultation in a culturally competent manner.
  • Assist in the accomplishment of the case management plan for the children and families under the supervision Assistant Program Director – VYS by screening, evaluation and identification of specific case management activities as identified in the individual service plan.
  • Adherence to reporting requirements for incidents of Child Abuse, Elder Abuse, Adult Abuse, Dependent Adult Abuse, and Tarasoff warnings per California reporting laws.
  • Document services to meet all clinical program and state requirements, noting significant intervention and progress toward development and attainment of a treatment plan.
  • Meet with and coordinate mental health services with TulareWorks staff for the purpose of reviewing client progress and to coordinate services for quality treatment planning.
  • Attend job related conferences and also participate in a variety of in-service training, seminars, and practicum.
  • Attend staff meetings as scheduled.
  • Attend weekly supervision meetings in any form that the Assistant Program Director - VYS directs.
  • Provide effective and thorough feedback to the referring agent for specific cases which have been referred to the program.
  • Provide direct services as outlined in the individual service plan the family.
  • Refer cases to the appropriate County Department of Mental Health Clinics and other appropriate resources as indicated.
  • Perform other tasks when asked by the Assistant Program Director – VYS.
  • Will drive on Agency business.
  • Will be able to think and act quickly and efficiently in emergencies.
  • Comply with all Turning Point Safety policies and procedures, including but not limited to: workplace safety, reporting work related injuries, Infection Control and preventing potential safety risks for staff, clients and others.
  • Punctual and regular attendance is an essential responsibility of each employee at Turning Point. Employees are expected to report to work as scheduled, on time and prepared to start working. Employees also are expected to remain at work for their entire work schedule. Late arrival, early departure or other absences from scheduled hours are disruptive and must be avoided.

ESSENTIAL JOB REQUIREMENTS

  • Bachelors’ degree in social work, counseling, or a related field. One (1) year experience working in a similar or related field.
  • Communicate effectively in written and spoken English.
  • Ability to communicate effectively in written and spoken Spanish is required.
  • Work effectively with diverse ethnic, religious, socioeconomic and professional groups of people.
  • Plan, coordinate, and facilitate services for mentally ill children, adolescents, and adults.
  • Screen and evaluate potential clients for the identification of case management/residential services and other activities.
  • Ability to operate within Electronic Medical Records (EMR) system.
  • Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint, Office 365, Teams, Zoom and other communication/meeting platforms.
  • Possess a valid driver’s license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
  • Ability to pass a criminal background check.
  • Ability to pass a pre-employment physical, drug screen, general physical, and TB test.
  • COVID19 vaccination required prior to employment.
  • Must maintain good relationships with current, future and past employees, co-workers, vendors, third party administrators, and others with whom the Agency is transacting business, and relate to them in a professional manner.

ESSENTIAL JOB REQUIREMENTS FOR BILINGUAL SKILL

  • Communicate effectively in written and spoken Spanish.
  • Required ability to communicate effectively with monolingual Spanish speaking clients and families for all clinical purposes.
  • Assist with phone calls for Spanish clients.
  • Required to provide language translations for case managers and Physicians on a periodic basis.
  • Other translation tasks as required.
