POSITION SUMMARY:

We are currently recruiting for a Project Services Coordinator. The Project Services Coordinator is responsible for providing case management services to veterans, active service members, and families of veterans. The position will conduct assessment of suicide prevention services needed, develop, and monitor service plans, establish linkages with appropriate agencies and service providers in Rhode Island to help participants obtain needed suicide prevention services, provide referrals to participants and related activities. This work will be accomplished by liaising between the RIDOH Center for Health Promotion and: RI’s Office of Veterans Services; RI’s Department of Behavioral Healthcare and Developmental Disabilities and Hospitals, the Providence Veteran’s Administration Medical Center, the Ocean State Research Institute and related duties as assigned. This full time position pays $26.00 per hour and is located in Providence, Rhode island.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
  • Perform casework duties with veterans or active service members who are risk of suicide to determine and address unmet resource needs.
  • Perform community engagement such as engaging program participants at community organizations that are easily accessible and visiting stakeholders at their respective organizations to create and maintain partnerships.
  • Provide culturally sensitive case management services for veterans, active service members, and their families that may include but not limited to screening for suicide risk, conducting individuals needs assessments, collaborating with the program participants to develop an individualized program plan.
  • To make creative use of available resources and enlist the cooperation and understanding of other social agencies for the meeting of unusual, unique and/or special needs or individual care.
  • Provide services to individuals who become at acute risk of suicide and therefore must be able exercise a wide degree of independent judgment, decision, and action on an ongoing, regular basis while knowing when to consult with care team.
  • Institute and revise assistance or program service plans promptly as needed, or to change treatment plan(s) and goals in accordance with the participant’s changing needs.
  • Re-evaluate the level of safety, the needs of and services being provided each time there is contact with the participant to ensure program effectiveness and cultural sensitivity.
  • Identify available services for the Veteran including mental health and other social determinants of health.
  • Network with our partners at the Rhode Island Office of Veterans Affairs and the Providence VA Medical Center.
  • Coordinate suicide prevention services with the Providence VA Medical Center’s Suicide Prevention team and other VA staff, utilizing RI Serves/Unite Us.
  • Collaborate with each veteran to create a care plan to address their goals and will utilize RI Serves for this purpose.
  • Become well versed in all policies, procedures, and guidance that RIDOH and maintain level of understanding and competencies.
  • Assess the level –and- urgency of need and appropriate support for all cases.
  • Apply active listening and crisis intervention techniques to cases in distress.
  • Utilize organizational and time management skills to effectively work in a fast paced and changing work environment.
  • Perform other duties as assigned by supervisor.
  • Participate in developing department goals, objectives, and systems.
  • Participate in staff meetings and attend other meetings and seminars as assigned.
  • Recommend new approaches, polices, and procedures, to continually improve efficiency of the department and services performed.
  • Efficiently performs duties as assigned and works as a team within division and company.
  • Perform other related duties as assigned.

Required Skills/Abilities:

  • Excellent verbal, written, and listening communication skills within the English language.
  • Proven, high quality customer service (internal and external customers).
  • Ability to work collaboratively.
  • Proficiency in a variety of counseling techniques and social work practices

  • Thorough knowledge of clinical social casework techniques
  • Skill in their application in collaboration with medical and para-medical professions, in the diagnosis and treatment of patients or persons to remove or alleviate social, emotional, attitude or environmental problems in order to promote their return to a normal and effective pattern of living
  • Working knowledge of community resources available for the use of patients or persons under care and the ability to apply such knowledge effectively as a member of a clinical team
  • Solid ability to analyze situations carefully and adopt appropriate courses of action.
  • Ability to understand and adhere to the duties, methods, and procedures required by the position.

Education and Experience:

  • Such as may have gained through graduation from a college of recognized standing with specialization in Human Service field of study (social work, counseling, psychology, or related field) Or, any combination of education and experience that shall be substantially equivalent to the above education and experience
  • Experience working with veterans and individuals experiencing homelessness preferred as may have been gained through employment in a responsible position involving hospital, public, business, or health administration
  • Experience conducting individual needs assessments, working with Veteran populations, and screening for suicide risk
  • Experience in conducting mental health screenings, psycho-social assessment
  • Experience creating individualized care plans
  • Experience in VA system in Rhode Island preferred
  • Rhode Island resident, and knowledge of Veteran’s Affairs systems preferred.
