Horizon Health in cooperation with the Substance Abuse and Mental Health Services Administration will plan, develop and implement a Certified Community Behavioral Health Center to serve Bell, Clay, Knox, Laurel and Whitley counties. We are seeking a psychiatric nurse practitioner to join our team effort in bringing much needed behavioral and mental health services to the service area. This is an exciting opportunity for a professional seeking to expand the scope of knowledge and lend expertise in the development of this exciting program.

The chosen candidate will work closely with the existing Horizon staff in providing services to established clients and families enrolled in our treatment and recovery program.

Candidates are expected to have a minimum of three-year experience delivering competent mental health services to individuals and families who are experiencing varying levels of difficulties due to life situations or mental health concerns. Here are just a few of the benefits of joining:

Responsibilities

  • Provide high quality, compassionate and ethical mental health therapy to clients
  • Consult with MD on medication management via telephone
  • Develop a treatment plan for each patient, using a patient-centered, harm-reduction model
  • Collaborate with on-site nurse practitioner and behavioral health counselors on care model
  • Complete clinical documentation
  • Maintain required client contact
  • Diagnostic Assessment
  • Maintain good standing with the appropriate professional board entities
  • Complete and maintain all required continuing education requirements for specific licensure and specializations
  • Demonstrate knowledge of current and emerging trends in techniques, issues and approaches in the mental health field
  • Be aware of community referral sources and refer when necessary
  • Maintain up to date case files within electronic health record (EHR) system
  • Utilize quality improvement data to enhance patient care
  • Maintain contact with and consulting with third parties and collateral contacts when appropriate.
  • Perform other duties as assigned and necessary for project reporting compliance

Requirements

  • Working knowledge of family systems and their impact on mental health within the family
  • Working knowledge of attachment styles and clinical presentation across the lifespan
  • Skill in applying varied interventions with individuals/families in crisis
  • Skill in working with a variety of cultures
  • Skill in working independently and implementing time management strategies
  • Ability to provide necessary self-care and demonstrate appropriate professional boundaries and behavior
  • Awareness of personal strengths and limitations
  • Hold a valid DEA

Job Type: Full-time

Pay: $135,000.00 per year
