Quality Improvement Specialist will assist the Quality Improvement Program Manager in accessing compliance with the agency’s quality improvement plan. The Quality Improvement Specialist will create PDSA’s, clinical reports and dashboards as requested by the management team to complete data requirements for the annual quality improvement plan, and to fulfill reporting requirements for the various grants and programs. This position will be responsible for assessing, improving, and encouraging data integrity to ensure accuracy of the reports and dashboards presented. They will be responsible for working across multiple departments within the agency to support quality improvement initiatives. This position provides support to the organization in the areas of Compliance and Quality Management. Areas of focus will include:

Quality Measures: Patient Outcome Data, UDS, HEDIS, meaningful use, and PCMHRevenue Cycle Management: Coding, Collections, and Billing performance / process improvement.

ESSENTIAL DUTIES AND RESPONSIBILITIES (Included but not limited to the following);

  • Assist Quality Improvement Manager in the implementation and execution of the total quality plan.
  • Assist Quality Improvement Manager in the writing of summaries and reports for presentation to staff, management, and board as related to the Quality Program.
  • Attend Quality Committee and meetings and manage the meeting notes and action plans as needed.
  • Initiate reports on a schedule determined by the QPM to monitor key performance indicators in clinical, financial, dental and pharmacy areas.
  • Initiate audits on a schedule determined by the QPM to identify areas of non-compliance.
  • Will create executive summary reports and dashboards to share reporting findings with management, staff and board of directors.
  • Assist management team in the coordination the preparation of the Annual UDS Report, coordinate response and explain results to management.
  • Support data requests and data feeds from CIMS and managed care entities.
  • Work with management and staff to create and implement quality improvement reports such as PDSA’s, Root Cause Analysis, Swim Lane Diagrams, and other reports related to clinical workflow or quality improvement.
  • Collect and review data related to operational quality measures to assist the Quality Improvement Programs Manager in monitoring business practice data. These measures include, but are not limited to patient cycle time, third next appointment availability, patient satisfaction data, and productivity.
  • Perform PSR/front desk record reviews focused on accuracy of sliding fee determination and other PSR measures.
  • Support the finance department activities by performing a periodic review of revenue cycle measures and preparing data reports.
  • Assist Compliance Managers and the Finance Team in continuing development and internal compliance review and monitoring relative to coding, billing and revenue cycle activities.
  • Works in conjunction with Quality Improvement Programs Manager to provide a summary and trending report periodically on all incidents, accidents and patient complaints. Reviews for compliance with applicable standards, regulations and laws related to safety and quality.
  • Monitors the patient outcome data continually, assessing that care is being delivered in accordance with established standards as indicated by Audit Schedule.
  • Work with staff and management from multiple departments on quality audits, assessments, training, and monitoring.
  • Will maintain provider specific, executive, and other dashboards in the reporting software.
  • Will provide other reports as requested by Quality Health Manager.
  • Understand the guidelines and requirements of Patient Centered Medical Home (PCMH) Accreditation and Meaningful Use and stay current on any changes that may be forthcoming and communicate these to the Administration.
  • Work with the EHR vendor to implement Meaningful Use/Promoting Interoperability (PI), UDS, HEDIS and other regulatory reporting features of the EHR and test the accuracy of the reports generated.
  • Maintain close communication with the administration on the status of compliance with Meaningful Use/Promoting Interoperability (PI) and any need for action on measures not meeting the standards.
  • Will become familiar with other reporting requirements to include but not limited to PCMH, UDS, and any other future entities.
  • Track patient satisfaction through practice management system and implement quality measures to track improvement.
  • Prepare reports for risk management meetings and attend meetings as needed.
  • Implementation of emergency preparedness and track safety/weather drills to comply with quality plan.
  • All other duties as assigned.
Requirements:


KNOWLEDGE, SKILLS AND ABILITIES

  • Advanced knowledge of eClinical Works (Electronic Medical Record System)
  • Familiarity with clinical care processes
  • Familiarity with interface functions, testing and maintaining, a plus
  • Familiarity with report-writing using EHR report writer, or another report writing software, or willingness to be trained on these
  • Project Management Skills
  • Writing and communication skills
  • Ability to create and present clear and understandable training and training materials

EDUCATION AND EXPERIENCE

  • B.S. in Healthcare related field, Clinical Informatics, or Computer Science, OR Public Health preferred
  • Experience with Quality Improvement processes in a healthcare setting.
  • Minimum 2 years’ experience in health care related field, preferably in a professional capacity OR extensive experience in a health care setting and strong working knowledge of the workings of electronic health record systems and it’s efficient interface with clinical processes including workflow and data analysis/report writing
  • Must hold all patient Protected Health Information (PHI) other patient personal information and agency information in confidence, in accordance with the attached Employee Confidentiality Statement, which I have read, understand and signed.
