The Clinical Manager establishes, implements, and evaluates goals and objectives for home care services that meet and promote the standards of quality and contribute to the total organization’s philosophy.

Essential Duties and Responsibilities:

  • Provides oversight of all client care including skilled nursing services, nurse delegation, medication administration and medication management per physician order and as appropriate based on state licensure.
  • Directs case conferences with care management team to ensure the best possible client experience.
  • Oversees the maintenance of client clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities.
  • Oversees the evaluation of organization performance via Quality Assurance Process Improvement program through quarterly and annual review.
  • Acts as facilitator, instructor and monitors care staff training, including initial orientation, performance of skills competency assessments, medication administration training, and compliance with quarterly and annual training program.
  • Oversees schedule of on-call nurses and participates in rotation of RN on-call duties.
  • Ensures compliance with all state licensure requirements and participates in audits with licensing and regulatory bodies.
  • Provides direct client care including but not limited to:
  • Performs client assessment upon admission, change of condition, and as required by state law.
  • Develops the plan of care and makes necessary revisions as client status and needs change.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client.
  • Performs supervisory visits to instruct, supervise, and evaluate personal care staff as required by state law.
  • Obtains/receives physician’s orders as required.
  • Participates in emergency management plan as directed by agency administrator.
  • Acts as Brand Ambassador for Right at Home while providing community education, in-services, and building relationships with community partners.

Knowledge, Skills, and Abilities

  • Bachelor’s degree in Nursing from an accredited program preferred.
  • Active Registered Nurse license to practice professional nursing in the state required.
  • Experience in a healthcare training setting preferred (CPR, AED, CNA, etc).
  • Current Basic Life Saving or CPR certification required.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
  • Ability to relate to clients and care staff in a way that is sensitive to the unique needs of the individual.
  • Competent computer skills and a familiarity with electronic medical records.
  • Ability to work in a discrete manner maintaining confidentiality of sensitive information; self-starter with ability to multi-task in a fast paced environment; excellent organizational skills and ability to prioritize work load and adhere to strict deadlines.'

Job Types: Full-time, Part-time

Pay: $40.00 - $50.00 per hour
