• Demonstrates regular, reliable and predictable attendance.
• Ensure LTSS Members admitted to nursing facilities or rehabilitative institutions are assessed timely for appropriateness to relocate to community.
• Ensure the review of completed health risk screenings, transition plans of care, and other assessments to determine appropriate level of referral and appropriate level of service delivery.
• Oversee coordination of all relocation activity including assessment, discharge planning and coordination with assigned case manager.
• Ensure timely entry of assessment data in identified systems.
• Serve as primary point of contact for nursing facility staff, relocation contractors, and other service providers for issues surrounding relocations.
• Level and assign to appropriate service coordination teams and/or disease management vendors within established timeframes.
• Attend medical necessity fair hearing.
• Conduct and collaborate a thorough and sound clinical and functional review of member’s needs with a focus on safety and quality of the care pre and post transition from the nursing home.
• Ensure compliance with all required reporting according to contract, regulatory and company policy guidelines.
• Able to travel up to 50% of the time.

Education/Experience: Bachelor’s degree in Social Work, Psychology, Gerontology, or Human Services related field or equivalent experience. 5+ years of related experience. Bilingual preferred

License/Certification:
LPN, RN, or LCSW required.
