Central Oklahoma Community Mental Health, Norman, OK is currently recruiting for a Behavioral Health Case Manager to work as part of the PACT team (Program of Assertive Community Treatment) The PACT Team serves no more than 95 clients. This position is a case manager providing home based services to clients with severe and persistent mental illness. The case manager functions as an integral part of the multi-disciplinary, PACT treatment team. The individual caseload will be from 12 to 18 clients. Job duties include but are not limited to: developing/maintaining recovery oriented treatment plans, leading individual and group psychosocial rehabilitation, providing case management by linking consumer to other community resources, and providing crisis diversion when necessary. Complete the appropriate documentation of services. May be assigned to act as a payee.

  • Full-time position
  • Annual Salary Range: $40,000 - $47,000 depending on education and certifications (plus a benefit allowance!)
  • FLSA Status: Non-Exempt
Minimum Qualifications and Experience:
  • Behavioral Health Case Manager: Bachelor's degree from an accredited program in a behavioral health field.
  • Senior Behavioral Health Case Manager: Bachelor’s degree from an accredited program in a behavioral and two (2) years of experience in professional case management behavioral health services.
  • Offer of employment is conditional on becoming a certified CM II within 6 months.

About this Facility: COCMHC was opened in 1968 and was the nation’s first community mental health center. It was funded through a federal grant program that has resulted in similar facilities throughout the country. Built on the premise that people suffering from mental illnesses would be better served in the community as opposed to an institution, the center has played a pivotal role in restoring the lives of countless area residents. COCMHC was awarded a CARF Three-Year Accreditation for the following programs: PACT (Program of Assertive Community Treatment), Evaluation and Referral, Adult Outpatient, Team Green PSR (Psychiatric Rehabilitation), Systems of Care and Transitional Services, and Child and Family Services. COCMHC is in Norman, Oklahoma and employs approximately 95 individuals from Norman and surrounding communities.

THIS AGENCY REQUIRES COVERAGE 24 HOURS, 7 DAYS PER WEEK. WORK HOURS AND LOCATION MAY VARY DEPENDING ON BUSINESS NECESSITY. EMPLOYEES MAY BE REQUIRED TO WORK WEEKENDS AND OVERTIME.
