Position: Shelter Project Coordinator

Salary: $60,000 to $65,000 per year (based on experience and credentials)

Location: Ronkonkoma, NY and travel throughout Suffolk County

Hours: 37.5 hours per week

Start Date: Immediately


Position Description/Key Responsibilities:

The Shelter Project Coordinator (Clinical Service Interventions to Homeless Persons Program) engages and supports chronically homeless individuals throughout Suffolk County. The Shelter Project Coordinator assists individuals with identifying appropriate, permanent housing or with resolving barriers preventing return to other desired environments, working closely with those served until stable housing has been achieved. The Shelter Project Coordinator serves individuals who have serious mental illness (SMI); services may include linkage to needed services, monitoring established goals and providing case-specific advocacy. The Shelter Project Coordinator will identify opportunities for client engagement and conduct outreach in venues that eligible clients may utilize such as homeless shelters, jail and forensic service offices, hospitals, and health/behavioral health centers. The Shelter Project Coordinator establishes supportive, helping relationships and works with individuals to achieve improved overall health and social functioning. The Shelter Project Coordinator oversees and partners with a Peer Navigator who utilizes personal, lived, recovery experience to engage and assist those served by the program. The Shelter Project Coordinator works closely with MHAW’s Adult Care Management team.

Qualifications:

The successful candidate should possess or be quickly able to acquire the following skills and knowledge:

  • Desire to work with chronically homeless individuals and compassion for those who have experienced the loss of a safe and dignified home.
  • Excellent inter-personal communication including the ability to establish empathic, empowering relationships with people who have experienced trauma, psychiatric and substance abuse challenges as well as chronic physical conditions;
  • Strong communication, organizational and computer skills.
  • Understanding of the key elements of recovery and wellness, including the role of family and self-help;
  • Basic knowledge of the public health/mental health system, public entitlement and social service programs;
  • Candidate must be a reliable self-starter with the ability to work both independently and as part of an interdisciplinary team.
  • Master’s degree in a human service/health discipline preferred and two years’ experience with assessment, outreach or delivering behavioral health services to those who are chronically homeless.
  • Supervisory experience.
  • Experience conducting community outreach activities and presentations.
  • Bi-lingual (English/Spanish) candidates are strongly encouraged to apply.
