Job description

The Social Worker is an integral part of the Hospice Interdisciplinary Care team and as such, is responsible for assessing the psychosocial need of the hospice patient and family. Understanding that psychosocial services must be delivered according to accepted standards of practice, the social worker, guided by the patient/family’s needs and wants, coordinates the resources of the family, the hospice, and the community into an integrated program of support.

Duties and Responsibilities

· Participates in evaluating referrals to assure appropriate admissions

· Implements hospice information visits and sign up visits as directed

· Assesses patient and family for psychosocial needs and concerns within 1 week of admission

· Participates with the interdisciplinary team in developing the team plan of care within 1 week of admission

· Coordinates psychosocial care to meet the needs and concerns of the patient and family in a manner that is meaningful to them

· Visits hospice patients/families when directed by the interdisciplinary team

· Acts as liaison where needed between patient/family, hospice team, and outside resource agencies

· Responds to patient/family need after hours when requested

· Coordinates and monitors Medicare and Medicaid reimbursement

Qualifications

· Master’s degree in Social Work from a school of social work accredited by the Commission on Accreditation of the Council on Social Work Education (CSWE)

· State certified as a social worker

· Knowledge of hospice philosophy, grief –loss, death and dying processes

· Valid New Mexico driver’s license, personal auto and current auto liability insurance

Knowledge and Abilities

· Knowledge and sound understanding of hospice philosophy and its concepts of care.

· Knowledge of and ability to apply psychosocial skills and counseling techniques in a wide variety of hospice situations

· Ability to respond and function appropriately in crisis situations

· Extensive knowledge of resource or support agencies and the method of accessing services

· Ability to conduct educational seminars and training for civic, church, volunteer and professional groups
