(*Sign on Bonus Eligibility is based on your status at time of hire. Status must be that of full time, working at least 30 or more hours per week for the $2500.00 bonus and anyone hiring in part time, working less than 30 hours per week would be eligible for the $1000.00 bonus. All sign-on bonuses are prorated and paid over 5 quarters.)

Performs direct resident care for each resident in his or her assigned area.

Licensure, Certification and Registration:

State Tested Nursing Assistant

CPR Certification

First Aid Certification


EXPERIENCE:

One year experience in Home Care/Acute Care/Long Term Care

Certified in CPR

PRINCIPAL JOB ACCOUNTABILITIES:

Must report for work on time and on a regular basis as scheduled including overtime.


1. Assist Residents with activities of daily living such as: Personal Hygiene (bathing, hair care, teeth, nails, skin care, and toileting.), Eating (feeding, observe intake and output and record as specified, assist Residents at meal time), Ambulating (assist Residents with cane, walker, prostheses or transport residents by wheelchair).


2. Provide everyday services, which include laundry, ironing, bed making, medication reminders, linen change, meal preparation, light housekeeping tasks, shopping, errands, sorting mail, clerical help, pet care (including dog walking), watering plants, cleaning closets, organizing, packing/unpacking/moving in-.house.


3. Assist in other areas to support resident’s well being, encouraging socialization, participation in activities, transporting resident to programs within the facility, companionship services which may include reading, walking, pet sitting. Accompany resident on outings or to medical appointments.


4. Communicate with all disciplines to provide quality resident care and report all unusual findings/changes to Charge Nurses, Director, Wellness, and/or Clinic Nurse.


5. Complete appropriate documentation in a timely manner.


6. Assist in maintaining a clean, pleasant and safe environment for all utilizing Universal Precautions.


7. Complete: 12 hours of in-service education as required by OBRA and Laurel Lake Retirement Community, attend monthly staff meetings, and mandatory meeting. Maintain CPR/First Aid certification.


8. Work in other departments as assigned: Crown Center, Greenwood, Rosewood, Clinic, Activities.
