Fraser is looking for Case Managers to work with individuals on the CADI/BI & DD waivers. Case Managers serve as an advocate to Fraser clients and connects clients to resources and services within the community. The ideal candidate will demonstrate compassion for clients, commitment to success and value relationships. We seek an individual who is very well organized, has experience with individuals with developmental or intellectual disabilities and/or mental health diagnoses and experience documenting and case noting.

Responsibilities:

CADI/BI WAIVER - The Case Manager provides client-centered case management services to individuals receiving CADI/BI waiver services. Case Managers typically have about 35 clients on their caseloads and meet with clients two to three times per year. The Case Manager is responsible for care coordination, monitoring health and safety needs of clients, monitor services to ensure they are effectively meeting the client’s needs and intervening as necessary. Caseloads are typically a majority of adults, with a small mix of children as well.

DD WAIVER & NON-WAIVER - The Case Manager provides client-centered case management services to individuals receiving DD services. Case Managers typically have about 43 clients on their caseloads and meet with clients two to three times per year. The Case Manager is responsible for care coordination, monitoring health and safety needs of clients, monitoring services to ensure they are effectively meeting the client’s needs and intervening as necessary. Caseloads are typically a mix of children and adults.

Location and Schedule:

Our Case Managers work Monday - Friday during standard business hours. Case Managers can work fully remotely after the first 3-6 months of onboarding is complete! Travel to meet with clients is required, client meetings may occur 2-3 times per week, although clients may choose a phone or video conference in lieu of an in-person meeting during the COVID-19 federal public health emergency. Case Managers typically meet with clients in their assigned county.

Requirements:

DD Case Managers must have one of the following:

  • At least a bachelor's degree in social work, special education, psychology, nursing, human services or other fields related to the education or treatment of persons with developmental disabilities or related conditions
    • If degree is in social work, social work licensure is required.
  • One year of experience in the education or treatment of persons with developmental disabilities or related conditions (QDDP qualified).
  • A minimum of one course that specifically focuses on developmental disabilities (Fraser can provide this course to you free of charge).

CADI/BI Case Managers must have one of the following:

  • At least a bachelor’s degree in social work, psychology, sociology or a closely related field.
  • At least a bachelor’s degree in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency.

Other Requirements:

  • Valid Minnesota Driver’s License with acceptable record.
  • Ability to pass DHS background study.
  • Effective communication in the English language, including verbal and written communication with both internal and external contacts.
  • Ability to perform basic mathematical skills, including accurate billing.
  • Ability to understand and implement all related regulations, policies and procedures.
  • Commitment to promoting diversity, multiculturalism, and inclusion with focus on culturally responsive practice, internal self-awareness and reflection.
  • Ability to recognize problems, establish facts, and draw valid conclusions.
  • Computer proficiency using Word and Excel with proficiency in keyboarding skills.
  • Excellent organizational skills and attention to detail.
  • Basic mathematical skills, including the ability to add, subtract, multiply, divide and calculate percentages.
