1. Bachelor’s degree in community health, dental/oral health, public health, or related field or equivalent combination of education and experience totaling 4 years.

Preferred Qualifications:
1. Fluency in Spanish, Somali, Hmong, Vietnamese, or Pashtu.
2. Experience working in a community dental care setting in a supportive role
3. Experience being self-directed in carrying out program goals.
4. Experiencing working with people from diverse ethnic and socioeconomic backgrounds.
5. Knowledge and interest in serving multi-culturally diverse patients with mental, medical, dental, sexual, and chemical health needs.
6. Knowledge of community resources and/or social service agencies.
7. Experience in providing care coordination to a complex, diverse population.

This position contributes to the efficiency and effectiveness of the clinic’s services by providing dental care coordination efforts for adults, children and families which also includes access to services and essential oral health education to people experiencing barriers to access care. This position provides dental care coordination services for CUHCC’s patients using an approach that is culturally tailored and supports the patient’s overall health as it relates to dental care, and management of chronic health conditions and social determinants of health.
REPORTING RELATIONSHIPS:
This position reports to Dental Clinic Manager.
ESSENTIAL FUNCTIONS
1. Provide Care Coordination services to patients receiving dental care at CUHCC 65%
- Follow Care Coordination procedures including, but not limited to, contact requirements, documentation requirements, and panel management approach
- Complete appropriate training and orientation for Care Coordinators
- Provide diligent outreach and engagement strategies
- Provide optimal access to services (e.g., attend to referrals in a timely manner)
- Data entry and management of patient registry for all assigned patients (e.g., intake/wellness assessment/oral health action plan completion dates, referral locations, internal/external care team, etc.)
- Coordinate patient’s care (e.g., schedule appointments, assist patient in preparing for appointments, provide appointment reminders, do check-in calls following appointments and/or provide no-show follow-up calls)
- Coordinate care with patient’s Care Team, including internal and external providers as well as patient’s family or identified supports (e.g., providing updates about changes, health care needs or concerns, ensuring continuity of care, completing necessary paperwork to access care)
- For all care coordination patients assigned, serve as the point person for patient’s dental care within the clinic promoting integrated care
- Participate in consultation and supervision with supervisor and team
- Participate in training, as requested
- Attend and participate in team meetings as requested
2. Basic education, outreach, and promotion of oral preventive care 25%
- Conduct patient outreach via phone and CareMessage texting service to bring current patients back to the clinic for preventive visits and follow-up appointments.
- Promote pediatric dental services via outreach activities and CHW services.
- Integrate patients’ dental-related goals and education needs into care plans.
- As requested by supervisor, work and support CUHCC’s initiatives such as Good Faith Estimates and others.
- Identify or develop dental education materials in relevant languages (videos, infographics) for CUHCC patients.
3. Other
- Participate in quality improvements efforts
- Create resource lists that compliment patient needs
- Support efforts to build and maintain key community partnerships
- Other duties as assigned
