Senior LIFE is currently seeking a Home Care Manager to fill a full time, permanent, position at Senior LIFE.

Responsibilities

Responsibilities of the Home Care Manager

• Assess and coordinate the homecare needs of the Senior LIFE participants and identifies and develops specific plans of care for them in combination with the IDT.
• Conduct supervisory visits to Senior LIFE participants in their homes and overseeing their approved care/services to assure quality service is being provided
• Conduct ongoing assessments to ensure the care plan in place is the best course of action
• Attending family meetings along with Senior LIFE team meetings to provide updates on participant statuses and needs
• Documentation, data collection and tracking of all care/services provided to participants

Qualifications

Requirements of the Home Care Manager

• Must have a valid Driver's License, vehicle and current auto insurance
• Clinical degree or relevant education in a health and human services related program and related experience in the field is required
• Experience working with the elderly population

Vaccination from covid-19 or exemption is a condition for hiring.
