Community Bridges is currently seeking a Jail Coordinator

JOB SUMMARY
The Community Bridges Inc. (CBI) Jail Coordinator is responsible for delivering treatment, and discharge planning services to identified Navajo County Jail Program Patients. Jail Coordinator is responsible for assisting inmates with jail release planning and coordinating into behavioral health services. Jail Coordinator will participate in a weekly staffing with probation and Jail staff to provide updates on each patient and coordinate care. Each are below is defined by the specific function as it relates to the policy, process, and the governing rules set forth by the Department of Health Services (DHS), Regional Behavioral Health Authority (RBHA), Arizona Complete Care (ACC) Office of Behavioral Health Licensure (OBHL), and the Code of Federal Regulations (CFR).

RESPONSIBILITIES
Each discipline is governed by specific policy and process. It is the responsibility of the employee to read and learn the associated policy and processes as they will be held responsible for carrying out their duties in a competent and ethical manner

Counseling Services

• Facilitates an individual discharge planning with each patient enrolled in the Navajo County Jail Discharge Program and determines need for on-going behavioral health services.
• Ensures that patients receive treatment services in a language and format that they can understand.
• Ensures that treatment received is based upon the needs and service frequencies identified and is in line with the goals outlined in the jail assessment.
• Ensures that patients are being treated based upon their current Stage of Change as well as their cognitive and mental capacity.
• Participates in designated clinical staffing sessions with probation and designated jail staff.
• Ensures that a Behavioral Health Assessment for each patient who accepts discharge planning and coordination services
• Ensures that an intake is scheduled for patient within 24 hours of release from jail.
• Other duties as assigned by supervisory and administrative support.

Discharge Plan
Responsible for development of an individualized discharge plan that may include MAT, Psych and Physical Health Practitioner induction.

• Addresses needs and objectives of each patient’s discharge plan in group, individual, and/or family counseling and education sessions and documents progress made towards objectives and/or areas of needed improvement and a plan of action to achieve improvement.
• Coordinates with probation, designated jail staff, and Behavioral Health treatment agency in order to identify progress, barriers, or required changes to the patient’s discharge plan
• Other duties as assigned by supervisory and administrative support.

MINIMUM EDUCATION & EXPERIENCE REQUIRE

• 2 years related experience in behavioral healthcare, physical health or substance abuse.

LICENSES, CERTIFICATIONS, CREDENTIALS, REGISTRATIONS

• Current AZ Driver’s License
• Associate or Independent Licensure is preferred but not required
• Must pass a background check and receive clearance to go into the Navajo County Jail.
