Summary:

Valley View Recovery Center in Cannon Falls, MN, a client-centered, holistic MI/CD facility, is looking for an LADC, ADC-T, or equivalent to facilitate groups and individual counseling for our 49-bed, high-intensity, residential substance abuse treatment program for adult men. This individual would work with a caseload of up to 8 clients.

Programming includes primary groups for substance use disorder, in addition to mental health groups, and alternative holistic therapies. Located on a beautiful campus, our clients are able to enjoy the fitness center and other rec. areas and activities. Recreation therapy is a huge part of what Valley View Recovery Center believes in. Our goal is to provide support to individuals recovering from substance use disorder through a holistic approach, helping them find joy and hope throughout their healing journey.

Essential duties and responsibilities include the following (other duties may be assigned):

  • Provide individual and group therapy, case management, crisis intervention, education, and support to clients
  • Perform and document client admission assessments, ISP, and IAPP
  • Collect information from collateral resources to aid in determining diagnosis and need for treatment
  • Creation and monitoring of individualized treatment plans and treatment plan reviews containing client’s goals, activities, and objectives of client care in a timely manner
  • Conduct ongoing evaluation of client needs to make appropriate community and in-house referrals
  • Document patient progress through counseling and interaction through groups
  • Attend clinical staffing meetings
  • Work closely with clients to engage them in the decision-making process and help them understand the benefits of assessment and accessing the appropriate level of care as indicated
  • Complete documentation as appropriate regarding authorizations, client needs, family needs and agency requests. Document all contacts, incidents, and progress while client is in program
  • Work collaboratively with all other organizational services and team members
  • Available to clients and referents for questions and other assistance.
  • Maintain ongoing, current, and required training for best practice service delivery
  • Ensure and maintain compliance with all chapter 245G and other state and program requirements
  • Responsible for obtaining and/or maintaining proper licensure and/or certification according to specific state requirements
  • Other tasks as assigned

Requirements:

  • Demonstrated knowledge and skills working with colleagues who thrive in a multi-cultural setting
  • Demonstrated ability and emotional intelligence to work in unpredictable, crisis response, and team-oriented setting
  • Maintain strict and confidential guidelines regarding current and former clients, their families, and referents in accordance with federal and state laws
  • Strong verbal and written communication skills
  • Self-directed and ability to work independently
  • Valid LADC or equivalent licensure in the state of MN
  • Familiarity with Procentive software a plus
