We are seeking a Licensed Practical Nurse that will work under the direction of a Registered Nurse Case Manager. A home health LPN will maintain a patient’s quality of life by following the assigned plan of care. Providing direct nursing care under a managed team of therapists and other professionals to treat patients/clients under a physician’s plan of care and in accordance with the policies and procedures of the Agency, compliance with the conditions set forth in the State Nurse Practice Act, and regulatory entities. The LPN goals are to rehabilitate and educate the patient to regain and retain independence in their home setting. A. provided care plan will include all clinical care, symptom control and identify other physical, psychological, social, and spiritual needs for assigned patients.

Key Duties Include:

  • Provide skilled nursing services in accordance with the orders of a physician and under a plan of care in a patient’s home.
  • Be responsible for the observation, assessment, nursing diagnosis, counsel, care, and health teaching of the ill, injured, or confined, and the maintenance of health and prevention of illness of others. (See Tactical Deliverables).
  • Follow Agency Policies and Procedures, Federal and State Regulations, and Standards of Practice.
  • Attend Agency In-services, Case conferences, or other meetings as requested.
  • Records the health needs of the patient.
  • Provides those services requiring substantial specialized nursing skills based on the physician’s orders and the nursing diagnosis. Examples of such services include, but are not limited to; physical assessment, patient and family teaching, urinary catheter insertion.
  • Foster a positive relationship with patients/caregivers and doctors to improve patient outcomes.

Education and Experience:

  • Currently licensed as a Practical Nurse to practice in the State of Arizona (or) Current multi-state license from another compact state utilizing their valid compact state license for up to 30 days until an Arizona permanent license is issued.

Job Types: Full-time, Part-time, Per diem

Pay: $30.00 - $35.00 per hour
