ACP's PASS Program helps families of children with special health care needs with daily life skills. Families in the PASS Program will work side-by-side with their clinical support team to support goals in: independence with daily living skill, safety skills, social and community functioning.
Requirements:Job Responsibilities
- Implements direct personal assistance goals developed by the family in conjunction with agency with the individual.
- Provide safe one to one care per the plan for children with special health care needs as directed by parents/caretakers.
- Provide socialization opportunities within home and/or outside of home when appropriate.
- Perform all necessary self-care tasks as needed by the individual supported.
- Documents progress on required data sheets and progress notes which are made available to family and clinical consultant
- Assists people with learning about and advocating for their personal rights and understanding the responsibilities that accompany those rights.
- Drives people to destinations, as approved by parent/caretaker necessary for them to experience a full and diversified life, following all safety and legal regulations.
Qualifications:
- Experience with children 1 year (preferred).
- High school Degree/ GED.
- Valid RI Driver's License.
Abilities
- Ability to stand, walk, or run (when applicable) for 2-5 hours at a time.
- Ability to lift children (approximately 20-50+ pounds), when necessary.
