Position Purpose:


Summary:

Provides medical social services and bereavement coordination aligned with the plan of care to Hospice patients and their families or caregivers in their homes. Provides outreach in the community regarding Evangelical Hospice services.

Qualifications:


Education:
Essential:

  • Requires a Bachelor's degree in social work from a Council on Social Work Education accredited institution OR a Bachelor's degree in psychology, sociology or other social work-related field and supervised by an MSW, OR with a Bachelor's degree from a school of social work accredited as above, AND hired in hospice before 12/02/2008 MSW supervision is not required.
  • One-year experience in health care required.

  • Requires BLS at time of hire or within 30 days of hire.

Credentials:

  • Pennsylvania Driver's License

  • Proof of insurability required. Required to have automobile for transportation

Pennsylvania state police criminal record check, child abuse history clearance and an FBI criminal history check required.
