Treatment Plan Specialist – Adult Group Home – 40 hrs/wk (900TPS)

Tuesday-Saturday (Flexible hours)

Salary: $47,000/yr

Qualifications:

• Bachelor’s Degree in Recreation or Human Service Field
• One year of experience working with individuals with Developmental Disabilities preferred

Key Responsibilities include, but not limited to:

• Work with individual residents and the IDT regarding scheduling, planning, implementation and supervision of all individuals' active treatment in the form of ADL/ILS goals
• Participate in Clinical IDT’s and complete reports on individual program plans.
• Ensure compliance with state and federal requirements pertaining to ADL/ILS active treatment programs, ICF regulations and life safety codes.
• Initiate and participate in in-service training.
• Assist direct care staff with ADL/ILS program implementation and treatment plans for residents.
• Complete observations of all staff implementing formal goals, ongoing services, and staff actions.
• Initiate and maintain appropriate communication with other clinical disciplines as appropriate to assure coordination and integration of services.
• Review and update Comprehensive Functional Assessment and Abilities Profiles.
• Perform any other duties as requested by the Executive Director or his/her designee.
