Position: Home Services Worker (HSW)

Reports to: PAS Supervisor

Job Summary: Provide supportive and personal services for the clients with consideration of dignity and privacy. Provide personal care and hygiene to personal assistance clients.

Qualifications/Educational Requirements:

1. Does not have a disqualifying background check under the requirements of the Health Care
Worker Background Check Act without a waiver;

2. Has a copy of the Social Security card; and
3. Has visa or proof of citizenship in compliance with federal requirements for employment.
4. Complete a minimum of eight hours of training prior to his or her first assignment or provide proof of successfully completing a training program at or through another licensed home services agency within the prior year (previous 365 days).

5. Graduate of an accredited High School or equivalent preferred.
6. Ability read, write and follow directions.
7. At least 18 years of age and has demonstrated competency.
8. Trained in first aid
9. Work positively and favorably with clients, families, and staff.
10. Demonstrate compassion, responsibility, and cheerful attitude.
11. Able to demonstrate competency in all areas of training for personal care.

Responsibilities/essential functions: The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.

1. Follows the instructions of the PAS Supervisor in providing personal assistance services.*
2. Provide assistance with activity of daily living support, personal care, medication reminding, housekeeping services, personal laundry, cooking, shopping, assistance in getting to and from appointments, maintenance of household records, and companionship.

3. Have knowledge of agency policy and procedures.*
4. Observe and report any safety hazards found in the client’s home or any significant observations regarding the client.*
5. Attend staff meetings, attend/complete 8 hours of annual in-services, and participates in orientation of new employees. *
6. Report client complaints to the PAS Supervisor. *
7. Be trained in first aid.*
8. Maintain client confidentiality/adheres to HIPAA requirements and agency policy and procedures manual. *
9. Possess the ability to follow written and oral instructions. *
10. Report abnormal findings in client’s conditions as observed and per service plan to PAS

Supervisor. *

Environmental Elements

Employee works in an office environment sometimes with moderate noise levels, controlled temperature conditions and sometimes travels to patients homes where they may have direct exposure to hazardous substances. Employees may interact with upset staff and/or public and private representatives in interpreting and enforcing departmental policies and procedures.

Employee will have to travel to a variety of client homes and perform in conditions that vary greatly depending upon the client’s home environment. Some homes will be clean, neat, and maintained at a comfortable temperature. Other homes may be cluttered, dirty, with an uncomfortable temperature.
