Patient Aligned Care Team RN Care Coordinator Job at Distinctive Home & Healthcare

Distinctive Healthcare is currently seeking to hire Full Time RN PACT to work at the The Ralph H. Johnson VAMC in (Charleston, South Carolina).
Responsibilities:
1.1.1.6. Keeps patient provider and appropriate case manager aware of
patient’s progress, issues, and/or problems
1.1.1.7. Screens patient for social service, home care, and other community
care needs; and coordinate or make referrals as appropriate and
seek consultation when indicated. He/she possesses the knowledge
and skills to effectively apply all aspects of the nursing process within a
collaborative, interdisciplinary practice setting to facilitate the effective
care of patients.
1.1.1.8. Agency nurses assigned to Charleston VA PACT Clinics can expect
to work 8-10 hour shifts.
Qualifications:
1.1.1.1. Years of Experience in the Specialty: Minimum 2 years current (must
have worked
within the last six months) experience in a primary care setting.
1.1.1.5. Minimum Education: Graduate of a school of professional nursing
approved by theappropriate State-accrediting agency and accredited by one of the
following accrediting bodies at the time the program was completed by the applicant: The
Accreditation Commission for Education in Nursing (ACEN) or The Commission on
Collegiate Nursing Education (CCNE). Bachelor’s Degree in Nursing or in related field or
a master’s degree in Nursing or in related field is highly preferred.
